Please provide a response for each question. A lack of response may result in your materials being delayed or not produced. Please take notice of the amount of time needed for materials- a lack of proper notice may result in materials not being created, distributed, or published. Any materials that you need printed should be attached to this form. Please do not email them separately or direct to LibraryAware. You will be asked to fill the form out again with your documents attached.
